Marketing materials often highlight polymer thickness in mils, arbitrary grip textures, or generalized powder-free claims while obscuring the foundational regulatory identity, FDA clearance status, and standardized barrier testing requirements.

To establish a verifiable purchasing file, buyers must specify medical gloves through a structured hierarchy: first verifying medical-device identity and FDA 510(k) clearance status, second matching the material formulation to the ASTM specification FDA still names in 510(k) review, third stating watertight-barrier criteria under 21 CFR 800.20, fourth enforcing the federal powdered-glove ban, and finally requiring chemotherapy permeation data only when that labeling claim is made.

Separate medical gloves from industrial gloves before naming a material

The United States Food and Drug Administration (FDA) regulates disposable gloves intended for medical use as medical devices under Title 21 of the Code of Federal Regulations. Current FDA oversight classifies medical gloves as Class I reserved devices. Unlike exempt Class I devices, Class I reserved medical gloves require premarket notification [510(k)] review and formal clearance prior to commercial distribution in the United States.

Medical gloves fall into three primary regulatory categories:

  • Patient Examination Gloves (21 CFR 880.6250): Disposable, non-sterile or sterile gloves worn on the examiner's hand to prevent contamination between patient and examiner. Product codes vary by polymer chemistry: LYY for natural rubber latex, LYZ for polyvinyl chloride (vinyl), and LZA for synthetic polymers such as nitrile (acrylonitrile butadiene) and polychloroprene.

  • Surgeon's Gloves (21 CFR 878.4460): Gloves intended to be worn on the hands of operating room personnel to protect a surgical wound from contamination. The current identification at 21 CFR 878.4460 is a non-powdered surgeon's glove, Class I, commonly coded KGO. FDA's Medical Gloves page treats medical gloves as Class I reserved devices requiring 510(k). They are typically supplied as sterile pairs. FDA's 2008 Medical Glove Guidance Manual still discusses design-control expectations for surgeon's gloves; confirm current quality-system duties against the live quality-system regulation rather than treating the 2008 manual as the powder-ban rule.

  • Chemotherapy Gloves (Specialty Examination Gloves): As established in FDA's Medical Glove Guidance Manual, gloves labeled for handling chemotherapeutic agents are classified as specialty patient examination gloves under product code LZC. The chemotherapy claim is an expanded labeling indication that requires validated drug permeation testing data.

In contrast, non-medical or industrial-grade gloves are manufactured for food service, janitorial maintenance, automotive work, or general industrial processing. Because industrial gloves are not intended for medical diagnostic or therapeutic procedures, they do not undergo FDA 510(k) premarket review, are not subject to medical device quality system regulations, and are not covered by the medical glove powder ban. Each named ASTM specification is a material-and-intended-use file for 510(k) review. Public catalog and guidance pages identify the specification; they do not authorize reprinting paywalled tensile, elongation, or thickness tables. Two additional test methods appear throughout FDA's glove 510(k) recommendations:

State the powder ban and the residual-powder distinction

A pivotal regulatory boundary in medical glove procurement is the federal powdered glove ban. On 19 December 2016, the FDA issued a landmark Final Rule (effective 18 January 2017) banning powdered surgeon's gloves, powdered patient examination gloves, and absorbable powder for lubricating a surgeon's glove.

The ban was codified in the Code of Federal Regulations under banned device sections:

  • 21 CFR 895.102: Banned powdered surgeon's gloves, defined as gloves intended to be worn by operating room personnel that incorporate powder for purposes other than manufacturing.

  • 21 CFR 895.103: Banned powdered patient examination gloves, covering disposable natural rubber latex, synthetic, and vinyl patient examination gloves containing added dusting powder.

  • 21 CFR 895.104: Banned absorbable powder for lubricating surgeon's gloves (cornstarch-based lubricating agents).

The FDA's decision was driven by extensive clinical evidence demonstrating that glove powder presents an unreasonable and substantial risk of illness or injury, including severe airway inflammation, hypersensitivity reactions, granuloma formation, and peritoneal adhesion in surgical patients, as well as aerosolization of natural rubber latex allergens.

Hospital contracts must distinguish banned donning powder from trace residual manufacturing powder. The current identifications at 21 CFR 880.6250 and 21 CFR 878.4460 state that a non-powdered glove does not incorporate powder for purposes other than manufacturing and that the finished glove includes only residual powder from manufacturing. ASTM D6124 is the residual-powder method named in FDA's glove 510(k) recommendations. Public 510(k) summary K171541 reports residual powder at or below 2 mg/glove for that named nitrile examination glove; that milligram figure is device-specific evidence, not a universal limit for every medical glove.

Put leak testing in the tender as 21 CFR 800.20 plus the named ASTM hole method

Watertight barrier integrity is the fundamental safety attribute of a medical glove. In the United States, regulatory sampling plans and adulteration thresholds for glove leakage defects are established under 21 CFR 800.20.

Under 21 CFR 800.20, FDA considers a lot adulterated when leakage defects in the tested sample meet or exceed the rejection number at a 1.5 AQL for surgeons' gloves or a 2.5 AQL for patient-examination gloves. FDA's glove guidance treats the 21 CFR 800.20 1,000 mL water-leak test and ASTM D5151 as barrier methods. Emergence of water from the filled glove is a watertight-barrier failure.

An AQL of 1.5 is not a promise that 1.5 percent of gloves are defective. It is the minimum quality FDA will accept when the agency samples lots for leakage defects under 21 CFR 800.20. That sampling method is not the manufacturer's entire in-process quality plan.

21 CFR 800.20 is the adulteration threshold FDA uses when it samples lots, not a license to invent tighter mill-specific AQL numbers. Tenders should require a 510(k) identity, the intended-use statement, and lot evidence that watertight testing was performed to the named method. A manufacturer's tighter internal plan, if claimed, must be documented for that device—not copied from another SKU.

Treat chemotherapy permeation as an add-on claim, not as clearance

Handling antineoplastic and hazardous drugs requires specialized barrier protection to prevent occupational dermal exposure. However, widespread confusion exists in hospital sourcing regarding what constitutes a genuine chemotherapy glove.

Under FDA regulations, there is no separate medical device classification for a chemotherapy glove. Instead, as outlined in the FDA Medical Glove Guidance Manual, chemotherapy gloves are classified as specialty patient examination gloves under 21 CFR 880.6250 (Product Code LZC). To obtain this clearance, the manufacturer must submit specific drug permeation and penetration data derived from actual testing against individual chemotherapeutic agents.

The recognized consensus standard for hazardous drug testing is ASTM D6978 (Standard Practice for Assessment of Resistance of Medical Gloves to Permeation by Chemotherapy Drugs). ASTM D6978 is a recognized practice for chemotherapy-drug permeation testing. It supports a specialty examination-glove labeling claim; it is not a substitute for medical-glove 510(k) clearance and it does not convert an industrial glove into an FDA medical glove.
